This guide provides information to practitioners on what information is reported by the practitioner and/or other sources, what information is verified by DOH, and whether the information is optional or mandatory. The guide also provides instruction on how to complete and update the practitioner profile. We ask that you distribute this information to your members and include it on your website and/or in your newsletter if possible. We will also be distributing a statewide press release.
